Portable Health Profile (PHP)
The Portable Health Profile (PHP) is a tool to assist you with receiving ongoing quality health care. The PHP includes basic health information and should be portable to enable you to have the appropriate information at each health appointment. This can ensure that your healthcare providers have more complete and accurate information than might otherwise be available. The PHP can lessen the fragmentation of care among healthcare settings and will likely decrease the risk of medical errors.
Once the form is completed, you can print a copy and bring it with you to medical appointments. You could also consider keeping a copy on a USB key or your mobile device for easy access.
