OCCUPATIONAL THERAPIST – COMMUNITY

​Part-time, Full-time positions available

Location: Pickering, Ajax, Whitby, Oshawa, Bowmanville, Newcastle, Beaverton, Sunderland, Cannington

Responsibilities:  

  • Provide community based Occupational Therapy services within the client’s place of residence 
  • Provide occupational therapy assessment (through interview, standardized and informal assessment techniques) in the areas of home safety, home modifications, equipment prescription, fall prevention, mobility, cognition, and stroke 
  • Develop and implement best practice and evidence-based occupational therapy intervention in collaboration with clients/caregivers 
  • Provide education to client/caregivers and link to community agencies/resources when appropriate 
  • Complete funding applications as applicable 
  • Complete and maintain documentation related to assessment findings, care plans, progress and discharge summaries according to CAR, the referral source and COTO standards 

PHYSIOTHERAPIST – COMMUNITY

Part-time, Full-time positions available

Location: Pickering, Ajax, Whitby, Oshawa, Bowmanville, Newcastle, Beaverton, Sunderland, Cannington

Responsibilities:  

  • Provide community based physiotherapy services within the client’s place of residence 
  • Perform physiotherapy assessment (through interview, standardized and informal assessment techniques) in the areas of geriatrics, neurology, orthopedics, palliative care 
  • Develop and implement goal-oriented treatment plans in collaboration with the client/caregivers 
  • Assign patient care plan, and provide supervision and monitoring of physiotherapy assistants when appropriate 
  • Provide education to client/caregivers and link to community agencies/resources when appropriate 
  • Complete and maintain documentation related to assessment findings, care plans, progress and discharge summaries according to CAR, the referral source and College of Physiotherapists of Ontario standards
