We offer a range of rehabilitation programs to promote health, safety and independence for children, adults and seniors. All of our care and services are provided by Rehabilitation Professionals in the comfort and privacy of your own home.

Physiotherapy

Rehabilitation & Reconditioning  

  • Develop a home exercise program with easy to follow instructions to help you progress and achieve your goals  
  • Teach strategies to help control pain after a musculoskeletal injury, fracture or joint replacement surgery  
  • Provide intervention including education to support self-management, manual therapy, and exercise therapy
  • Participate in regular therapy sessions to help you return to baseline function and mobility after a period of immobility

Falls Prevention  

  • Gait and vestibular training to improve balance and protective reaction to help reduce falls  
  • Detailed assessment to identify goals and target areas of weakness contributing to falls  
  • Progressive home exercise program to improve overall strength and confidence

Mobility and Devices  

  • Assess and prescribe mobility devices  
  • Gait and mobility training sessions  
  • Care and maintenance of mobility equipment  

Aging Well  

  • A home exercise program to help counteract the impacts of aging by maintaining your mobility, strength and range of motion  
  • Promote an active and healthy lifestyle by providing recommendations and modification of activities

Occupational Therapy

Positioning & Mobility Devices

  • Assess and prescribe positioning and mobility devices
  • Training for independent use of mobility devices
  • Care and maintenance for positioning and mobility devices

Home Safety and Accessibility  

  • Home Safety Assessment to evaluate and identify safety risks to promote safety in the home environment  
  • Recommendations to maximize independence, minimize risk of falls or other serious accidents 

Therapeutic Surfaces and Pressure Management   

  • Identify causes of skin breakdown and make recommendations to protect the skin and promote wound healing  
  • Provide surface recommendations, training on proper use and maintenance of therapeutic surfaces  
  • Teach positioning / transferring techniques to decrease risk of tissue breakdown  
  • Develop and implement an individualized skin care plan, which may include a positioning schedule  
  • Provide caregiver education on wound management and factors affecting healing

Caregiver Education and Training  

  • Facilitate a strong partnership between client and caregivers to maximize independence and well-being  
  • Provide training on safe transfer techniques to ensure proper body mechanics and use of equipment  
  • Give tools and strategies for caregivers to care for, motivate and support their loved ones through daily activities and routines, disease management and encourage positive lifestyle changes  
  • Offer recommendations to help manage appointments, medications and change in lifestyle

Rehabilitation Assistants

Our Rehabilitation Assistants follow a treatment program designed by the Occupational Therapist or Physiotherapist. This allows for consistent rehabilitation opportunities.


Mobility & Seating

A Physiotherapist or Occupational Therapist can conduct a Mobility and Seating Assessment to determine what equipment may be required to maximize mobility and independence.

Our therapists, along with an approved vendor, can prescribe appropriate mobility equipment to improve your quality of life.  

We can assess for several mobility devices such as: 

  • Walkers  
  • Manual wheelchairs
  • Power wheelchairs  
  • Wheelchair back support or seating accessories  
  • Scooter

Registered Dieticians

Our team is equipped to assess and develop nutritional plans to meet nutritional needs and to provide education to promote healthier whole food choices and eating habits.  

Our Registered Dieticians can:

  • Create individualized nutritional plans that meets fluid and energy requirements  
  • Assist with pre-surgical optimization/post-operative nutrition  
  • Address concerns related to the effects of medication on nutritional intake  
  • Provide recommendations for palliative care needs regarding comfort eating, symptom management and ethics of feeding  

Social Workers

Our Social Workers can:

  • Offer counseling for individuals and support clients in crisis  
  • Assist with funding and housing applications  
  • Develop and review service plans, help navigate the health care system  
  • Serve as a liaison between hospitals, agencies, individuals and their families
